A DBQ is a form, not a product.
Public questionnaires live on VA.gov. Your own clinician can fill one. VA will not reimburse that bill. The exam they schedule is free.

Disability Benefits Questionnaires are the checklists clinicians use to describe a condition the way the rating schedule expects. VA publishes the public versions. Anyone can download them. That is a PDF, not a membership.
Your own provider can complete one and submit it. VA’s own claim-exam page is blunt: they will not pay or reimburse any costs related to your own provider completing a DBQ. If a shop is selling “DBQ packages,” they are selling a form you can print.
The exam VA schedules is the free one
If VA needs more, they set a claim exam — VA clinician or contractor. Tests they order are at no cost to you. A private DBQ can still be useful evidence. It is optional. It is not required to start 21-526EZ.
Do not wait on a paid package to file. Intent to file locks the date. The form is on VA.gov. An accredited VSO can help you attach what you already have.
