Buy CHCBP when TRICARE ends
The Continued Health Care Benefit Program is a premium bridge for 18–36 months after you lose TRICARE or TAMP. You must buy it within 60 days. DD Form 2837 to Humana Military with 90 days paid up front. Recut from TRICARE.mil. I will not file it.

Confirm TAMP or TRS actually ended
CHCBP follows TAMP, TRS, TRR, or other TRICARE loss. It is not a substitute for TAMP. If DEERS still shows TAMP, you are early.

Watch the 60-day clock
Purchase within 60 days of loss of TRICARE eligibility. Coverage starts the day after you lost TRICARE. Miss the window and this door closes.

Complete DD Form 2837 and pay the first 90 days
Mail it to Humana Military, Attn: CHCBP, P.O. Box 740072, Louisville, KY 40201-7472, with proof of eligibility and payment in full for the first quarter. Unremarried former spouses also send the divorce decree. Recut from TRICARE.mil CHCBP purchase page.

Know what it is not
Same shape as TRICARE Select, including pharmacy. No space-available care at military hospitals except emergency. No dental. Premiums are quarterly. Renew before the notice expires or you lose CHCBP permanently.
